When life is stressful or distressing, it can be hard for children or teens to take care of their mental health. Use our wellbeing plan template to help them get through the difficult days.

What is included in this wellbeing plan?
The wellbeing plan is 2 pages for you to fill in with your child, and then keep to look back and reflect on to help during stressful patches in the future.
How do I use this wellbeing plan for children?
Simply download the wellbeing plan template, print it off and fill it in with your child. It helps them to reflect on what causes them stress, how it affects them, and how to notice the signs that they are overwhelmed.
The second page of the plan helps your child find ways to self-soothe, and get support from others when they need it.
What can I include in a personalised wellbeing plan for my child?
On page one of the wellbeing plan template you can write in with your child the types of events and issues that cause them to feel distressed. You can help them think about how they can prevent stress and limit how it affects them, too.
They'll also be able to reflect on the signs that show them they're not feeling good mentally, and how those signs impact them.
On page 2 you can fill in practical steps that they can take to take care of themselves, and how others can help them, too.
What is a wellbeing plan for children and young people?
A wellbeing plan is a document that you can complete with your child, which serves as a guide for them to regularly check back in on. The plan helps them recognise when they're feeling stressed and what they can do to manage their feelings and feel better.
